Era el máximo dirigente de la Cosa Nostra siciliana hasta su detención en 2006.Su carrera mafiosa y escalada criminal comenzó en la década de 1950, cuando junto a Salvatore "Totó" Riina y Calogero Bagarella, se convirtió en el lugarteniente del gran jefe de la mafia corleonese Luciano Leggio.
